Vision Care Benefits Policy Section Name: Employee Relations Effective Date: January 1, 2022 Section Number: 400 Date of Revision: Policy Number: Page: 1 of 2 Subject: Vision Care Benefits 1. Purpose: The purpose of this policy is to designate the vision care benefits that will be made available for all regular full-time non-union employees and appointed officials of the City of Monroe. 2. Statement of Policy: 2.1 Effective January 21, 2022, the City shall provide regular, full-time employees (and their eligible dependents 1) vision care benefits, subject to such terms, conditions, exclusions, limitations, deductibles, co-payments and other provisions of the plan. Coverage shall commence on the 91st calendar day of continuous employment. 2.2 When on an authorized unpaid leave of absence of more than two weeks, the employee will be responsible for his benefit costs for the period he is not on the active payroll. Employees electing to continue such benefits shall pay the full cost of such continued benefits. Proper application and arrangements for the payment of such continued benefits must be made at the Human Resources Department prior to the commencement of the leave. If such application and arrangements are not made as herein described, the employee’s vision benefits shall automatically terminate upon the effective date of the unpaid leave of absence of more than two weeks. 2.3 Except as otherwise provided under COBRA, the employee’s vision care benefits shall terminate on the date the employee goes on leave of absence of more than two weeks, terminates, retires, or is laid off. Upon return from a leave of absence of more than two weeks or layoff, an employee’s vision care benefits plan shall be reinstated commencing with the employee’s return to work. 1 Eligible participants include the employee, the employee’s legal spouse, and employee’s children to the end of the month in which they turn 26. 2 An employee who is on layoff or leave of absence of more than two weeks or who terminates may elect under COBRA to continue at his own cost the coverage here provided. The Employer reserves the right to change the carrier and/or the manner in which it provides the above benefits, provided that the benefits are equal to or better than the benefits outlined above. 3. Definitions: None 4. Application: This policy shall apply to all non-union employees of the City, unless modified by an employment agreement and/or the written terms of appointment. 5. Responsibility: The Human Resources Director shall have the responsibility of implementing and overseeing this policy. 6. Administrative Procedure: None 7.
